PMP Dr. Tudericiu Vlad-George, hereinafter called DentaVis, provider of medical imagistic services, considers that any ownership, transfer and processing of personal data should be done according to the legal provisions of the EU Regulation 2016/679 and wishes to assure you that it will respect your right to confidentiality regarding your personal data.
The personal data processed by DentaVis protection policy follows the following principles:
● Always ensuring the confidentiality of personal data
● The data collection will be done in a transparent and legitimate manner
● Any information exchange will be made in an explicit manner and for the soul purpose which was brought to your knowledge
● All individual information, which we process, store or transfer will be adequately protected against unauthorised access
● Respecting the individual’s rights and will, regarding access, modification or deleting their personal data
Data collection within the DentaVis – Patient relationship:
1. The Patient’s last name and first name
2. The Patient’s gender
3. The Patient’s date of birth
4. The Patient’s address
5. The Patient’s e-mail address
6. The name of the prescribing physician
7. The type of radiography
8. The Patient’s radiography
9. The Patient’s signature (on the consent form)
10. The Patient’s ID series and number (it only applies to Patients requesting an invoice)
DentaVis will store this data in it’s systems for a period of at least 5 years. If, within 5 years, the Patient did not return to any of the DentaVis clinics, his / her data may be archived or deleted.
Reasons for personal data collection:
1. The Patient’s last name, first name and date of birth are collected in order to create the Patient’s folder and to be able to easily identify the Patient in our database, should he / she return to any of the DentaVis clinics to have other imagistic investigations done. The new imagistic investigations would then be attached to the Patient’s folder, thus making possible the displaying of the Patient’s medical history, ensuring homogeneity and, also, making it easier for the Patient and the doctor, should the Patient need a copy of his / her radiography. DentaVis commits to store the Patient’s medical investigation for a period of minimum 1 year or any other period required by law.
2. The Patient’s gender and age are required so that our operators can set the necessary radiation dose in order to obtain the appropriate result and to be able to submit quarterly reports to the Public Health Authority – Radiation Hygiene.
3. The Patient’s address is collected so that a fiscal receipt can be issued. According to the law, in order to draw a receipt, the company’s / Patient’s data (last name, first name and address) are required.
4. The Patient’s e-mail address is collected only if the Patient wishes to have the radiography in a digital format, this being optional.
5. The name of the prescribing physician is recorded, upon Patient’s request, in order to be able to send him / her the radiography. Please note that DentaVis will only send the imagistic investigation result to the prescribing physician for which the Patient gave his / her written consent and will, only upon Patient’s written and signed request, send the above result to other mentioned physicians.
6. The type of radiography is useful in order to know what kind of radiography to execute.
7. The Patient’s radiography. DentaVis commits to store the Patient’s medical investigation for a period of minimum 1 year or any other period required by law. After this period, the radiographies will be either archived or deleted, depending on the available IT solutions at that point in time.
8. The Patient’s signature as a result of him / her signing the informed consent form to have the imagistic investigation done and as a result of him / her signing the informed consent form to have his / her personal data processed.
9. The Patient’s ID series and number are only requested should the Patient request an invoice. In order to have this issued, the company’s data (should the patient represent a legal entity) and the delegate’s (the person receiving the invoice) data (last name, first name, address and ID series and number) are required.
According to EU Regulation 2016/679, you have the following rights:
● The right to access your data
● The right to intervene on your data (request rectification, update, block or delete incomplete or inexact data)
● The right to object to your data being processed
● The right to be informed
● The right not to be subjected to an individual decision
● The right to alert the authorities and / or the court, should you consider that your rights are not being respected
Also, starting May 25th 2018, you will have the following additional rights:
● The right to request us to restrict the processing of your data
● The right to data portability
Data security
Storing your personal data is our responsibility and DentaVis ensures you of the enforcement of adequate measures against disclosing, losing, destroying and leaking of your personal data, as well as of the fact that no unauthorised person will have access to our database, which contains your personal data.
